Introduction
This release focuses on intelligent automation, real-time data visibility, enhanced MIPS/ACO compliance, and mobile-first improvements to empower providers, streamline documentation, and deliver a more responsive care experience across the PacEHR platform.
New Features
Artificial Intelligence: Encounter creation with integrations (Enhanced Data Pull)
When creating an encounter, the system automatically fetches and displays allergies, medication lists, labs, and vitals from the past 60 days. This saves time for providers by pre-populating encounter fields and reduces data entry errors while improving clinical context. This feature lowers a typical encounter time from up to 10 minutes to less than 2 minutes of actual time.
Artificial Intelligence: Summarize Notes
Providers can now quickly generate summaries of a patient's recent encounters using AI. The system extracts key details from the past 10 encounters, consolidating relevant content into a structured, editable format. This improves efficiency during documentation, especially regulatory, annual wellness, and discharge events, saving hours of research for each provider.
Artificial Intelligence: Recent Activity [Patient Screen]
Recent Activity is a real-time patient activity log that consolidates updates from PacEHR, MatrixCare, PointClickCare, and future HIE Integrations. It prevents data duplication and highlights any new events including vitals, immunizations, medications, Admissions, Transfers, Discharges to hospitals, lab results, and location updates. This activity will save time by allowing the user to see all events in one simple to consume screen.
Tasking System: Centralized Tasking system
A task manager module has been introduced, giving providers visibility into pending, current, and upcoming tasks for each patient. Tasks appear on the dashboards and are tracked for completion. This encourages proactive documentation and improves care team coordination. Soon we will be adding Artificial Intelligence and creating tasks for users based on rules. Such as: If a lab result comes back from a facility, a task can be created to review the labs.
Telehealth Platform: Phone System (Voicemail, routing, mobile and web)
Providers can make and receive voice calls directly within PacEHR on both mobile and web platforms. These calls utilize a secure routing method, and the calls can be recorded. The phone system will allow the user to have a business phone number, instead of using a personal line, that is fully transferable to other members of the team if the user moves to a new facility. In addition, the calls can be routed to different users to support on-call rules and after-hours support. The calling system also brings in a new contact list function and fully supports call logging, voicemail, and recent calls facilitating quick communication with staff or patient family members.
Telehealth Platform: Video system (Waiting Rooms, mobile and web))
Integrated video calling now supports secure (HIPAA) group or individual video sessions. Providers can launch calls directly from the PacEHR application, and nurses /or patients can initiate a meeting room by going to a new video website. This allows virtual visits and remote care collaboration between care teams and families while continuing to document and review patient details inside of the PacEHR application.
Communication Platform: Secure Faxing (Advanced Routing, Real-Time notifications)
Fully integrated secure fax platform with patient data syncing with PacEHR, ensuring accurate details are available for faxing. Signatures, time stamping, return faxes are all supported. Enterprise support for larger groups for advanced routing by incoming fax numbers.

Artificial Intelligence
Exclude Invalid CPT Codes
The AI documentation assistant now ignores inappropriate or billing-invalid CPT codes (e.g., 99987). Only codes that can be submitted for billing (e.g., 99309) are included in AI-generated notes, improving coding accuracy and claim acceptance rates.
Dashboard Filters Fixed
Search and filter functions on the AI dashboard have been fixed and enhanced. Providers can now sort patients by name, location, room number, POS. This streamlines patient management and supports faster navigation to open tasks.
Rounding Note Deletion
Providers now have the ability to delete previously saved (but not signed) rounding notes. This allows corrections or clean-up if a rounding note was created in error or prematurely, preventing clutter and confusion in the patient's chart.
AI Prompt Editor – Phase 2
Admins can now create and manage custom AI prompts tied to documentation templates. Prompts can be system-level (default) or organization-level (custom). This ensures providers see relevant AI suggestions based on encounter context and documentation needs.
MIPS & ACO Measures – Decile Scoring & Benchmarking
Updated benchmarking logic has been applied using decile-based scoring. Each quality measure in the settings tab now displays the maximum points achievable, helping users understand the value of each measure. Measures are also sorted in ascending order for ease of review during signing encounters and setting up configuration.
Medical Director Activity Log Edit/Delete
Users can now edit or delete activity log entries within 90 days of creation. After 90 days, deletion is allowed only by the Provider of Record or Admin, with password authentication. This balances data control with audit trail integrity.
Floor, Bed, Room, and Unit in Dashboard & Reports
The AI dashboard and My Patients screen now shows the patient’s floor, room, bed, and unit location. These details are also included in exported reports, enabling better rounding organization, especially in larger facilities with multiple wards.
System Enhancements
Annual Wellness Visit Due Dates (AWV)
CPT codes G0438 and G0439 now automatically set the correct due date for Annual Wellness Visits (AWVs). For those utilizing Saisystems RCM platform; The integration will now auto update the Annual Wellness Due date based on an Eligibility check.
ICD Code Selection and HCC Data improvement – Enhanced Risk Scores
The system now allows for extensive Descriptions utilizing both the ICD Code and the Snomed information. We also updated the HCC Logic for those in Value Based programs to fetch updated HCC scores, displaying current and potential RAF values in real time. This helps providers understand documentation gaps and improve coding to ensure proper risk adjustment reimbursement.
Patient Attachments – Add Button & Filtering Improvements
An “+ Add Document” button has been added to the Attachments tab to improve file categorization. New filters and sorting options help users easily locate and manage uploaded documents, preventing misfiling and ensuring correct association with patient records.
Patient Clinical Data (Source Colum Added)
A new Source column has been added to Medications, Vitals, Problem List, Immunizations tabs and other tabs where we are importing data. This shows the origin of each data entry PacEHR, Facility (e.g., PCC/MatrixCare), or HIE—based on priority. This enhancement improves data traceability and helps providers validate the source before acting on the information.
Patient Medication Deduplication on Facility Re-Link
If a patient is re-linked to MatrixCare or PCC, the system avoids duplicating medications already in the chart. This prevents provider confusion and ensures medication history remains streamlined and accurate.
New Patient Update: Full Data Sync when adding a new patient
When a new patient is added from MatrixCare or PCC, a full data sync now occurs. This resolves previous issues where newly added patients had blank or missing fields, ensuring a complete and accurate patient profile from the start.
LTC Scheduler – “Completed Date” Field
A new field has been added to indicate the completion date of an appointment or encounter. This field appears in the patient banner and helps support workflow tracking, scheduling analytics, and billing follow-up processes.
Administrative: Facility Editor – Updated Email Labels
To eliminate confusion, email fields in the Facility Editor have been renamed for clarity:
“Facility Administrator Email”
“Secure Email Delivery”
“Facility General Email”
These changes improve communication setup and role-based email delivery.
Mobile Enhancements
Communication Module – Web & iOS
The communication module has been stabilized across web and mobile platforms. Real-time push notifications, alerts, and direct messages are now reliably delivered to iPhone, iPad, and web, improving care team communication.
Bug Fixes
User Image Reset – Bug Fix
When a user removes their profile photo, the system now correctly displays the default placeholder image. This resolves broken image issues and ensures consistent user interface presentation.
Medications Displayed as Active, but with ‘End Dates’
The logic for active medication filtering has been updated. Medications with an end date now display under the “Active” toggle until their designated end date passes. This ensures accuracy in medication reconciliation during encounter documentation.
Patient Census Screen – Incorrect Display of Last Signed Encounter Date
The “Last Signed Encounter” column incorrectly showing the verified date instead of the actual Date of Service (DOS), misleading providers about the patient’s most recent visit. The “Last Signed Encounter” column now accurately reflects the Date of Service for the most recently signed encounter, improving clinical oversight and patient tracking.
LTC Scheduler – Addition of New Follow-Up Protocols
New scheduling protocols for 120-Day Follow-Up and 6-Month Follow-Up have been added to the LTC Scheduler. This enhancement supports consistent follow-up tracking for long-term care patients.
History Section Data (Previous Encounter) in New Encounter
The logic has been corrected to display the most recent signed encounter date, regardless of which provider signed it. The history section now consistently reflects the latest patient data.
Allergies Not Transferring to PacEHR from External Sources
This data mapping issue has been resolved. Selected allergens now correctly populate the patient chart, ensuring complete allergy documentation and clinical safety.
LTC Scheduler – Schedules Not Displayed Upon Initial Load
This loading bug has been fixed. Now, upon logging in, the LTC Scheduler immediately loads the provider’s default schedule without requiring additional navigation.
Electronic Prescribing - Diagnosis Code Visibility
Diagnosis codes pulled from Point Click Care now appear under the “Diagnosis” section in DrFirst. This integration improves medication reconciliation and clinical documentation accuracy.
"No Data Available" Message Standardized
A standard message ("There is no data to display") now appears across mobile and web platforms in screens where data is not yet available (e.g., Encounters, Problems, Calling, Vitals). This improves UI consistency and reduces confusion.
