Release Highlights 

• Chronic Care Management (CCM): Structured workflow, unified dashboards, productivity reporting, and auto-open encounters. 

• Advanced AI Chart Auditing: Advanced rule-level filters and cleaner dashboards. 

• MIPS Quality Measure: Added support for Quality ID 126 – Diabetes Mellitus. 

• RAF Score Report: Nightly automated reporting for accuracy and performance. 

• Bug Fixes: Immunization duplication, provider license report dates, iPad usability. 

New Chronic Care Management System 

Workflow & Documentation 

Patient List (Census Screen) 

• When CCM is enabled, the Enrollment button launches the redesigned workflow. 

• Once enrolled, the button changes to “Open CCM”, giving direct access to chronic condition management, goals, and activity logging. 

Patient Screen 

• Centralized CCM documentation for abstractors, billing practitioners, and care teams. 

• Add/manage chronic diagnoses, view and update goals, and document care plan interactions in one place. 

CCM Dashboard 

• New unified dashboard consolidates eligibility, enrollment, activity tracking, and billing status. 

• Includes bulk operations, advanced filters, and actionable insights. 

• Enables care teams, billing staff, and admins to monitor CCM more efficiently. 

CCM Productivity Report 

• Consolidates all CCM activities (enrollment, billing, time logged, tasks performed). 

• Provides practitioners and administrators a full view of performance and workload. 

Advanced AI Chart Auditing 

Enhanced AI auditing with rule-level and dashboard filters. 

• Scope audits by location, place of service, visit type, templates, date ranges, ICD, and CPT codes. 

Dashboard improvements: 

• Displays both active and inactive rules. 

• Excludes providers without audited encounters for cleaner views. 

Benefit: More precise audits with focused results. 

MIPS Quality Measure 

Added support for MIPS Quality ID 126 – Diabetes Mellitus: Diabetic Foot & Ankle Care, Peripheral Neuropathy – Neurological Evaluation.

RAF Score Report  [Risk Adjustment Factor] 

Nightly job generates RAF scores without affecting system performance. 

• Ensures accuracy while maintaining optimal efficiency. 

Bug Fixes 

Resolved PCC Immunization de-duplication issues. 

Corrected Provider License Report date errors. 

Fixed iPad usability issue where General Notes were hidden behind the keyboard. 

New Integrations with Yardi!
